ORDER INSTRUCTIONS

1. FILL out this form by midnight the Friday before.
2. SEND
   1. CASH: Exact amount in an envelope with Child’s Name, Grade, Dollar Amount. (We cannot make change)
   2. CHECK: Payable to ‘Blessed Sacrament Catholic School’ with your oldest child.
3. TEACHERS will be collecting money for pizza in the classroom.
   – DO NOT TAKE CASH/CHECK TO THE FRONT OFFICE
